*URGENT* Home Care Nurse (Palliative Care, Office Hours, Islandwide)

Responsibilities • Make home visit to patient home • Provide chronic disease, activities of daily living and/or palliative advice and counselling, as well as nursing procedures such as nasogastric tube insertion, catheter management and wound care. • Write case report and update sytem on patient condition • Liaise with relevant personnels and tap into resources to provide for patients' needs • Provide emotional support for patients and family members Job Requirements • Min.
