*URGENT* Palliative Nurse (5 days Office Hours) #HRY

Job Responsibilities • Provide holistic and compassionate nursing care to patients receiving home care and palliative care services. • Conduct nursing assessments and monitor patients' physical and psychosocial conditions. • Develop and implement appropriate nursing care plans based on patients' needs. • Manage symptoms and provide appropriate palliative nursing interventions. • Administer medications and treatments in accordance with doctors' orders and established protocols. • Provide wound care, pain management, and other nursing procedures where required. • Educate patients, caregivers, and family members on disease management, medication, and end-of-life care. • Work closely with doctors, allied health professionals, caregivers, and other members of the multidisciplinary team. • Provide emotional and psychosocial support to patients and their families. • Maintain accurate and timely nursing documentation and patient records. • Participate in care coordination and follow-up to ensure continuity of care. • Adhere to professional nursing standards, infection control procedures, and organisational policies. • Perform other duties as assigned. Requirements • Registered with the Singapore Nursing Board (SNB) . • Valid BCLS certification . • Relevant palliative care experience is required. • Relevant palliative care certification / qualification is required. • Strong communication and interpersonal skills. • Compassionate and patient-centred approach to care. • Able to work independently and as part of a multidisciplinary team. • Comfortable providing care in a home and community setting.
