Perm Physiotherapist - Palliative

Responsibilities: • Manage clinical caseload and support palliative rehabilitation practice (e.g. breathlessness management, prescription of equipment, collaborative goal setting) across the various care settings. • Deliver evidence-based individualized care through holistic comprehensive assessment, treatment and evaluation of care. • To work collaboratively with multi-disciplinary colleagues to meet patient needs. • Participate in service review to drive service improvement. • Contribute to the implementation and monitoring of adherence to relevant clinical guidelines. • To be a resource and support to other team members and community professionals. • Take initiative in pursuing life-long learning and personal development through reflective practice and engage actively with learning and development opportunities. • Supervise and assign appropriate treatment plans to therapy assistants. • Maintain high quality patient documentation and records. • Any other duties as and when assigned.
