Health Coach – Community Wellness & Active Ageing #HDC1

Make a meaningful difference in community health and wellness. We are looking for a Health Coach to support community members in adopting healthier lifestyles, managing chronic health conditions and achieving sustainable wellness goals. This is an opportunity to combine your knowledge of health and wellness with strong people skills, while contributing to preventive healthcare and active ageing initiatives across the community. What You’ll Be Doing • Conduct health and lifestyle assessments to understand individual health needs, goals and risk factors. • Engage community members in meaningful health conversations and facilitate personalised goal-setting using the S.M.A.R.T. framework . • Develop and implement individualised health coaching plans based on wellness objectives. • Provide guidance and encouragement to help individuals overcome lifestyle and behavioural challenges. • Support community members in building sustainable healthy habits and improving their overall well-being. • Monitor, track and document progress, engagement and health outcomes. • Collaborate with the Community Health Team to coordinate holistic care plans and ensure effective service delivery. • Conduct health and wellness programmes at Active Ageing Centres and various community locations . • Participate in occasional weekend community events and outreach activities when required. What We’re Looking For • GCE A Levels / Diploma and above . • Diploma in Nutrition, Dietetics, Health Science, Sports Science or other relevant healthcare-related disciplines and above . • Background in Nutrition or Dietetics will be advantageous. • Strong communication, interpersonal and community engagement skills. • Confident in engaging with individuals and building positive rapport. • Good organisational, coordination and time-management abilities. • Able to work independently while contributing effectively within a team. • Comfortable travelling between different community locations. • Proficient in Microsoft Office and general computer applications.
