Temp Virtual Assistant Care Coordinator | Up to $3,500 #HHLT

📍 Jurong 🕒 Mon–Thu: 8:30am–6pm | Fri: 8:30am–5:30pm 💰 Up to $3,500/month 📅 8-Month Contract 💼 Responsibilities • Reach out to eligible patients and support enrolment into the programme • Monitor the patient dashboard and review responses received through the patient-facing chatbot • Route and escalate patient concerns to the clinical team according to established workflows • Follow up with non-responsive patients and guide them on using the chatbot • Verify patient enrolment and maintain accurate patient databases • Coordinate appointments, referrals, handovers and communication between patients and clinical teams • Document patient interactions, outcomes and escalations accurately across IT systems • Collate and validate data on enrolment, response rates, outreach, escalations and patient outcomes • Analyse trends, care gaps and non-response patterns to improve patient engagement and operations • Prepare reports and support audits, service evaluation and continuous improvement initiatives 📝 Requirements • Min. diploma in Healthcare Administration, Care Coordination, Nursing Support, Population Health or related discipline • Patient-facing healthcare, telehealth or chronic disease management experience advantageous • Familiarity with electronic medical records and multidisciplinary healthcare environments advantageous • Comfortable communicating with patients and caregivers via phone and digital channels • Able to identify concerns and escalate according to established protocols • Comfortable using Excel, dashboards and multiple healthcare IT systems • Strong coordination, organisation and attention to detail • Able to handle confidential patient information appropriately • Comfortable working independently and within a clinical team 📩 Email your resume to biomedical@recruitexpress.com.sg or Telegram @alexgohhl. ➖ Alex Goh Hock Leong CEI No.: R1763413 Recruit Express Pte Ltd (Healthcare & Life Sciences) EA Licence No.: 99C4599
