Care Services Coordinator [WCAN]

• Develop and review individualised care plans for residents based on their identified care and support needs, in accordance with the Home’s policies and procedures. • Implement the agreed care plans and monitor residents’ progress, documenting relevant observations and follow-up actions. • Plan, coordinate and implement appropriate programmes and activities to support residents’ care needs, well-being and quality of life. • Prepare six-monthly review reports and maintain monthly interview records for approximately 50 residents.
