Part-Time Patient Liaison Assistant (Healthcare) #HZO

Location: West Salary: $18/ hr Working Hours: Weekdays 8am - 2pm, Weekends 8am - 11am Job Scope: • • Engage patients and caregivers through counselling and outreach • • Explain programme requirements and care processes clearly • • Verify patient contact and residential information • • Address patient and caregiver concerns and facilitate programme admission • • Escalate complex cases or concerns to the clinical team when required • • Work closely with intake colleagues to ensure a smooth transition into the service • • Maintain accurate records of counselling sessions and outcomes • • Support process improvements and other ad hoc duties Job Requirements: • • Min.
