Social Work Associate (Home Care)

• To send whatsapp messages daily to new HC patients and their MSPs to inform them to complete the HC Patient Distress Screener. • To collate the responses on the HC Patient Distress Screener daily. • To refer to HC PSS the list of patients who scored ≥ 5 and to follow-up on those patients with scores of ˂ 5 on their psychosocial and spiritual needs. • To make follow-up phone calls and/or home visits to patients who scored ˂ 5. • To record all assessments and interventions into Ingo TPCC. • To attend IDT meetings if and when needed. • To provide monthly reports on the scoring of the HC Patient Distress Screener. • Any other duties as and when assigned by the HOD and/or supervisor Requirements • Diploma in Social Work, SocialServices, community development related disciplines, preferably with 1-2 yearsof experience in the health and/or social service sector Skills / Experiences: • At least 1 years of relevant experience in a healthcare environment preferably For interested candidate, please email your resumes to denyse.tan@recruitexpress.com.sg Denyse Nicole Tan Shuqin (#HDT) Recruit Express Pte Ltd CEI No: R1333503 Company EA license : 99C4599
